• I, hereby acknowledge and give consent for my child to participate in the First Presbyterian Summer Arts Camp. As lawful consideration for permitting me (my child) to participate in this activity, I hereby release and discharge First Presbyterian Church, its officers, employees, agents, volunteers and members of the Board of Trustees from all action, claims or demands for any injury or damages resulting in participating in this activity. It is also my responsibility to transport my child to and from First Presbyterian Church, Cartersville.
  • Drop off is at 8:30 and Pick up is at 12:30. We will be providing a snack for your child during the morning. On the last day of camp we will have a matinee performance at 1:30. That day we will be providing lunch to your child. Everyday we will be having movement and art, so please send your child in clothing that they can get messy and is appropriate to move in. 

    Fee: $75 per child for the entire week. 
    Due no later then: June 5th 2019

    Please write a check to First Presbyterian Church or go to the church website and make a donation- please specify what the donation is for. Mail in or bring in this form to the office. If other arrangements need to be made, please contact us directly.
